Please complete all parts of the following employment application and release form completely and accurately. Once your application bas been reviewed, we may contact you to schedule an interview. You do not need to call back.

I understand that Great Western Lumber will only consider my application active for 90 calendar days from the date of my application. I understand that if l want Great Western Lumber to consider me for a longer period of time then I must complete and file a new

Great Western Lumber is committed to providing a safe and productive work place free from the presence of drugs and alcohol. As part of the post-application, pre-employment process, Great Western Lumber will require a urinalysis exam, which detects the use of illegal drugs and alcohol. If the test results are positive, the applicant may at their own expense pay to have the test redone. If it is negative, then the applicant will be reimbursed the cost of the test. If the test repeats positive, or otherwise altered, then Great Western Lumber will not consider me for employment.

I authorize Great Western Lumber to conduct a complete investigation of my prior employment record for the last 5 years. I understand that this investigation will include a reference check with some or all of the employers whom I have identified, as well as an effort to verify the fact that I have provided complete and accurate information.

request by Great Western Lumber:
